RELEASE OF LIABILITY & TERMS OF SERVICE
I accept for use as is the equipment listed on this form and accept full responsibility for the care of this equipment. I have made no misrepresentations to Stick Docs LLC regarding my height, weight, age or riding ability.
I understand and am aware that snowboarding and skiing is a HAZARDOUS activity. I understand that the sports of snowboarding and skiing and the use of this equipment involves a risk or injury to any and all parts of my body. I hereby agree to freely and expressly assume and accept all risks and injury or death to the user of this equipment while snowboarding and/or skiing.
I understand that the snowboarding equipment being furnished forms a part of or all of a binding system which will not release at any time or under any circumstances and that its use cannot guarantee my safety or freedom from injury while snowboarding and/or skiing. I further agree and understand that the binding system does NOT REDUCE THE RISK of injuries to any part of my body. I agree that I will release Stick Docs LLC from any and all responsibility, or liability for injuries or damages to the user of the equipment listed on this form or to any other person. I agree not to sue or to make a claim against Stick Docs LLC or the manufacturer or the distributer of this equipment for the injuries and/or damages relating to snowboarding and skiing and/or the use of this equipment. I AGREE TO RELEASE Stick Docs LLC of any negligence arising from the maintenance, transportation, mounting, tuning, selection or adjustment of this snowboard or skis.
I hereby agree to accept the terms and the conditions of this contract. This document constitutes the final and entire agreement between Stick Docs LLC and the undersigned. There are NO WARRANTIES expressed or implied and this snowboarding and or skiing equipment is accepted “as is”. I have carefully read this agreement and release of liability and fully understand its contents. I am aware that this is a release of liability and a contract between myself and Stick Docs LLC and I sign it at my own free will.
I agree that I understand how the ski-boot binding system works, and I have been instructed in its proper use. Any questions I have had about this equipment have been fully and satisfactorily answered. I agree that the binding release/retention setting numbers appearing in the visual indicator windows on the binding correspond to the binding release/retention settings recorded on this form. I agree that I have received the manufacturer’s written usage instructions (for new equipment only). I agree to check this ski equipment before each use, including the Teflon pad (AFD) under my ski boot sole, and that I will not ski if any parts are worn, damaged, or missing. I have read, understood and fully agree to the term and conditions set forth in this form.
I or other persons will be present at the time of pick-up or drop-off, otherwise I agree to pay the “missed drop-off” fee of $10.
